In the current way, pressing the icon on either of the rows will add a new field at the bottom.

I think that the icon next to the phone number should be linked to the phone number. maybe a call button.

One icon for adding new fields next to the field name i feel is more intuitive.

I think the plus icon next to the field name could be the way to implement this across all the components.

Initially, I moved it out of the top row because of the logical flow, particularly for keyboard input. Tabbing out from the last text input should then go to the option to add new. Since tab flow normally goes left-right/top-bottom, I thought it would be good to have it inline.

However, I agree with your comment about use of space. We could actually set the tab order so that tabbing out of the last input actually goes up to the add icon inline with the label. I think the one thing I would do is to move the add icon to align to the right/end of the label line. That keeps it in a consistent and easily locatable position for all components with multiple values.
