EHR data will be a file for a patient with a lot of columns and timestamps - and each timestamp will have an entry only in one of the columns. This is the simplest way to represent the data - based on the discussion between @skeating , @jeremyestein and Claire.

@skeating to try to get a file into DSH with a corresponding data that we can change before we do this for real.
