Ralph loop recipe doesn't create IMPLEMENTATION_PLAN.md

I have tried the Typescript and Python ones. They seem to run fine, but no IMPLEMENTATION_PLAN.md is generated. Changing the prompt to more explicitly ask to generate it doesn't work either. Maybe a permission problem?
