Refocus does not deploy to Heroku even using defaults

I'm simply trying to deploy an instance of salesforce's Refocus app by clicking the "Deploy to Heroku" button from the very top of https://opensource.salesforce.com/refocus/docs/03-quickstartheroku.html
(for a default setup).

I use all the defaults and just enter a valid app name and click the "Deploy app" button.
Please see attached screen shots. The red exclamation mark icons in the 3rd screen shot provide no additional info (not clickable / no tooltip).

Getting the same errors. Something has changed on the Heroku side that is not allowing the scripts to complete.
