Fix so disabled controls are totally ignored from built up object model

Given a form containing

<input type="hidden" name="x" value="hello" />
<input type="hidden" name="x" value="hello" disabled="disabled" />

form2js will currently return a null value for x - disabled controls should be totally ignored and instead form2js should return {x:"hello"}
