Add Ramp plugin wrapper and MCP config

Summary

• add a Codex plugin manifest for the Ramp plugin folder
• add Ramp MCP configuration pointing at the hosted Ramp MCP endpoint
• wire the existing Ramp skill into the plugin wrapper for discovery in Codex

Notes

• used Ramp's hosted MCP endpoint at https://mcp.ramp.com/mcp
• did not add an app manifest because this integration is MCP-backed
• the existing vendor-analysis skill appears to target an older Ramp MCP surface and may need a follow-up update to match current tool names

Validation

• validated plugins/ramp/.codex-plugin/plugin.json with jq
• validated plugins/ramp/.mcp.json with jq
