Add instant command execution with modifier keys

Implements instant command execution when selecting text while holding a modifier key (Shift, Ctrl, Alt), bypassing the menu popup.

Changes

Core Detection

• Added useDetectInstantCommand hook that listens for mouseup events with modifier key pressed
• Auto-executes configured command when text selection detected
• Mac compatibility: maps Ctrl to Meta key

Notes

• "Last executed command" feature from original spec deferred to keep PR focused
